Vaccination and relevant documentation

GENERAL CONDITIONS RELATED TO VACCINATION FOR U.S. VISA MEDICAL EXAM:

VACCINE REQUIREMENTS ACCORDING TO APPLICANTS AGE:

ADULTS VACCINATIONS REQUIRED FOR US VISA MEDICAL EXAM:

  • Tetanus, diphtheria (booster if necessary)
  • Covid-19
  • Pneumococcal (65 years and older)
  • MMR (measles, rubella, mumps)
  • Hepatitis B (under 60 years old; over 60 years vaccination not required)
  • Varicella (it is sufficient to provide a medical history of this illness during the medical exam)
  • Flu (for this year)

*Refugees and K1 visa applicants do not need a complete vaccination.

FAQs on vaccination

  • MAY I REFUSE ANY REQUIRED VACCINE?

The list of vaccines that are required for U.S. Immigrant Visa purpose has been set by the Centers for Diseases Control and Prevention (CDC) and My Clinic cannot change it. If an applicant’s vaccine history is incomplete and the applicant refuses a single dose of any required vaccine that is medically appropriate for the applicant, it should be documented that the vaccine requirements are not complete and that the applicant refuses vaccination. If the applicant does not intend to apply for an individual waiver and is not the subject of an adoptee exemption affidavit, the applicant is inadmissible to the United States.

  • IS A CONFIRMATION ABOUT THE COMPLETED VACCINATION FROM MY DOCTOR ENOUGH?

No. The applicant has to submit all available written records of vaccination history for review. Acceptable vaccination documentation must come from a vaccination record, either a personal vaccination record or a copy of a medical chart with entries made by a physician, or other appropriate medical personnel. Only those records of vaccine doses that include the dates of receipt (month, day and year) are acceptable. Self-reported vaccine doses without written documentation are not acceptable.

  • DO I NEED TO TRANSLATE MY VACCINATION RECORDS?

We accept vaccination records written in Roman characters. If your documentation is recorded in other languages (for example in Cyrillic, Asian or Arabic), it must be translated into Czech or English language for our staff to understand. We accept a common translation, there is no need to seek a legal one.  

  • WHAT IF I DO NOT HAVE A WRITTEN VACCINATION RECORDS?

We accept only a written vaccination records as the proof of your vaccination history. If you are not able to provide your vaccination documentation, you should be tested for a complete panel of the relevant antibodies. You can do so in My Clinic on a fee-for service basis. According to laboratory results you will get a recommendation about missing vaccinations and a number of necessary doses from our panel physician. The missing vaccinations can be applied in My Clinic by a nurse prior to your medical exam by the panel physician. You can also get tested or vaccinated by your personal paediatrician/general practitioner or in any vaccination centre and bring the written documentation as the proof.

  • WHERE I CAN GET VACCINATED?

You can get vaccinated anywhere – by your personal paediatrician/general practitioner, or in any vaccination or travel medicine centre. However, in such case you should bring a written record to your medical exam as the proof. 

The missing vaccines could be administered also in My Clinic on fee-for-service basis during or after your medical exam according to the indication of the panel physician. In case mentioned above, you may get vaccinated by a nurse prior to your medical exam as well if you know which vaccine you need.

  • MAY I SEND YOU MY VACCINATION RECORDS PRIOR TO MY MEDICAL EXAM FOR CONSULTATION?

No. Due to our capacity reasons we are not able to consult your vaccination records prior your medical exam. Your vaccination records will be reviewed by the panel physician during your medical exam.

  • IS COVID-19 VACCINE ASLO REQUIRED?

  • WOULD YOU GIVE ME ANY CONFIRMATION ABOUT MY VACCINATIONS?

Yes. After your medical exam and vaccination review by a panel physician (or after the administration of missing compulsory vaccines at My Clinic or any other vaccination center), we will give you the copy of your “Vaccination Worksheet or DS-3025 form.” It will become your official vaccination book for your stay in the United States. If all required vaccination conditions have been met, the document will be given to you in a hard copy by the panel physician during your medical exam. If needed to complete your vaccination requirements, we would have to wait until it´s done and close your health case documentation after that. It might, therefore, take some time to issue the Vaccination Worksheet. In such a case we could send it to you electronically if pre-agreed, or you may come to pick it up personally.
